For the last five years, a delegation led by Professor Aram Smolinsky, director of the Department of Cardiac Surgery at the Sheba Medical Center, has annually visited Perm, on the Siberian border, to operate on children with heart diseases. Although the cultural life in the district of Perm is highly developped, people do not have even one department for pediatric heart surgery. As the Russian health system is not equipped to cope with the problem, several hundreds of newborn babies and children with severe congenital heart anomalies pay the heavy price every year : some die a short time after birth and others struggle in the shadow of death. About one thousand children require urgent heart surgery but only eight operations can be performed within the week the delegation spends at the local hospital. The choice is made daily and the most severe cases are selected. The movie follows the marathon of operations, the tension, the anxiety, life balanced on scales, hovering between despair and hope; the team of doctors struggling with medical, moral and ethical questions raised by the need to choose.
